Transcript Requirements for MPH, MHA and Regents' Certificate Students

If you are in the process of completing a degree, we must receive an official transcript showing the degree conferred before you may register for classes in your program.

Please have the official transcript(s) sent directly from the college/university to the address noted below

Transcript Requirements for MS and PhD Students

As an MS or PhD student, you program is jointly administered by the School of Public Health and U of M Graduate School. The Graduate School requires all matriculating students to provide official tanscripts/academic credentials from each college/university from which they have received a degree in order to register for classes. Note that we do not receive official transcripts from SOPHAS. Please have transcripts sent directly from the college/university to the Graduate School at:
