How to Transfer Field Experience Credits

Option 1: LCS 690 Field Experience 

Before the field experience:

1.   Complete the MSU Course Agreement Form for LCS 690 and give this to Dr. Bartlett. 

2.   Complete the U of MN petition form and attach the completed MSU Course Agreement Form for LCS 690 and any written course assignments.

• On the petition form, the field-site mentor’s signature is NOT needed, just the MSU advisor’s signature.
• Ask your MSU advisor (LCS 690 course moderator) to sign under “Instructor” on the U of MN petition form.

3.  Make a copy of the petition form and submit the original to Kate Hanson for processing.  The original signed petition form and completed MSU Course Agreement Form will be attached to your MPH study plan when you prepare to graduate from the University of Minnesota, School of Public Health.

Option 2: PubH 7296 Field Experience

Before the field experience:

If the field experience is solely designed to fulfill the field experience requirements of the SPH, complete the online field experience contract module prior to the experience. The purpose of the contract is to avoid any misunderstandings between you, the preceptor and advisors.

After the field experience:

Complete the online evaluation form after the experience.  Both the student and preceptor will complete an evaluation.  Then the academic adviser will review them and, if approved, assign a grade.
