Primary care presents a critical focal point for improving the health of individuals with chronic diseases. New models of primary care delivery that increase coordination, emphasize prevention, and enhance collaboration are being widely adopted. Hear Dr. Carlin talk about a project that leverages access to a sample of electronic health record data that captures elements of optimal diabetes care (ODC) for nearly all adult patients with diabetes in the state of Minnesota (United States of America).
