Forms for Planning

HSRC has created some forms for client use when planning to work with HSRC in research support. We hope that these forms

Counting Questions

A key factor in creating an estimate/bid is the number of questions in the survey or interview. Find out how HSRC counts questions.

will assist you in identifying the services needed and/or bids for the support work you are planning to use from HSRC in your research study/project. At this time, if you need an estimate for planned work, you will need to copy the form needed, fill in as much information on your study and the work you would like HSRC to perform for you and send that information to HSRC.

HSRC will expand the number and nature of forms available to you for defining the nature and level of research support needed for your research work. We are working on building some interactive tools to improve the sizing and bid processes for work requested. For a detailed estimate, questions or assistance, please send your study information and contact HSRC.

Descriptions

General Study Estimate -
Here are tools for obtaining a bid from HSRC for a study or project for a mailed survey or a telephone survey. Select the tool that is most appropriate to your study. These tools are documents that assist you in identifying the characteristics of your study or project with enough detail that HSRC is able to provide you with a bid for the work scope. With these documents you would copy and paste the document into MS Word and fill in the applicable blanks. Then return the worksheet to HSRC for estimating purposes.

Note these estimate documents can be used for phone surveys, or mailed (self administered) surveys, and studies that are a combination of these work needs and survey modes. HSRC will contact you for additional information related to your request for an estimate, if needed.
