Application to the University of Minnesota School of Public Health is though SOPHAS (Schools of Public Health Application Service). SOPHAS is a service of the Association of Schools of Public Health, which represents the 40 CEPH accredited schools of public health.

University of Minnesota Supporting Documentation Requirements for Admission

All supporting documentation listed below should be provided directly to SOPHAS (except where noted).

1. Statement of Purpose and Objectives
Provide an essay in SOPHAS describing your past education, experience, and current professional career objectives. You are encouraged to comment on any or all of the following: plans you have to use your education and training; the needs and/or challenges you perceive as important in your field of study; and any personal qualities, characteristics, and skills you believe will enable you to be successful in your chosen field of study.

A. Applicants to the Ph.D. program in Health Services Research, Policy and Administration should gear their statement toward their preferred Area of Emphasis.

B. Research Statement - Epidemiology Ph.D. applicants only. Epidemiology Ph.D. applicants must also provide a proposed research statement directly to the School of Public Health.

2. Résumé or C.V.
Upload directly in the appropriate section of the SOPHAS application.

3. References
Provide three letters of reference from persons qualified to assess your academic work; clinical, public health, or professional experiences; or leadership potential in public health. Letters of reference should be provided directly to SOPHAS from the recommenders.

4. Official Transcripts: U.S. Institutions
Have an official transcript sent from every U.S. college/university attended directly to SOPHAS, including institutions where courses are not required for admission or transferred to another institution. This includes non-degree program. (e.g., summer school).

5. Official Transcripts: International Institutions
Applicants who have attended international institutions should provide an official course-by-course credential evaluation by World Educational Services (WES) directly to SOPHAS. Applicants submitting WES credential evaluations are urged to first create the SOPHAS application. Once applicants have their 11 digit SOPHAS ID they will be able to use the WES online process created specifically for SOPHAS applicants.

Note: U.S.applicants who have studied abroad as part of their coursework at a U.S. university/college, do not need to provide an evaluation of their coursework taken abroad as long as the international coursework and grades are noted on their U.S. university/college transcript.

*Standardized test scores are not required of applicants to the noted major programs who have completed a doctoral-level degree at a U.S. institution of higher learning.

  • GRE  - Institution code "6874", department code "0616"
  • MCAT Designation: SOPHAS
  • TOEFL Designation: 5688
  • GMAT - Institution code: 6891 or choose "University of Minnesota - Twin Cities - School of Public Health - Healthcare Administration" from the drop-down list

For the following tests, please have official, certified or attested copies of test scores sent directly to the University of Minnesota, School of Public Health:

  • LSAT
  • DAT
  • ECFMG

7. English Language Proficiency

Applicants whose native language is not English or whose education was completed exclusively at an institution(s) whose language of instruction is not in English must prove English proficiency by providing proof of one of the following:

  • Official TOEFL scores - An official report of the scores should be sent directly to SOPHAS. Designation: 5688. TOEFL scores must be under two years old.
  • Official IELTS scores – An official report of the scores should be sent directly to SOPHAS. Designation: SOPHAS
  • Completion of 16-semester credits/ 24-quarter credits (within the past 24 months) in an academic program at a recognized institution of higher learning in the U.S. or Canada
  • Educational Commission for Foreign Medical Graduates (ECFMG) certificate

8. Entry Terms and Application Deadlines

Applicants should be sure to note the appropriate entry term(s), priority (if any) and final deadlines noted below for submitting a complete application for admission (including the application fee in the correct amount and all required supporting documentation – noted above). Deadlines are for the subsequent noted term.

Priority deadlines (where applicable) are for School of Public Health scholarship opportunities and admission consideration.

Note: Applicants should be sure to submit their SOPHAS application not later than 11:59 p.m. Eastern Time on a particular deadline date.
