Regents' Certificate Application for Admission

Thank you for your interest in the University of Minnesota, School of Public Health (SPH). This application for admission information is for applicants to the following Regent’s Certificate programs only:

  • Public Health Certificate in Food Safety and Biosecurity
  • Public Health Certificate in Preparedness, Response and Recovery
  • ISP Certificate of Management Studies in Health Services Administration (Courses I, II and III)

Application Fee

An application fee of $75.00 must accompany each application for admission. The application fee cannot be deferred and is not refundable. It should be sent in the form of a personal check or money order made payable to the University of Minnesota. International applicants should submit the fee in the form of a bank draft drawn on a U.S. bank, made payable (in U.S. dollars) to the University of Minnesota. Credit card or cash payments are not accepted.

Provide an essay describing your past education, experience, and current professional career objectives. You are encouraged to comment on any or all of the following: plans you have to use your education and training; the needs and/or challenges you perceive as important in your field of study; and any personal qualities, characteristics, and skills you believe will enable you to be successful in your chosen field of study.

Provide an up-to-date résumé or C.V.

Letter of Recommendation

Provide one letter of recommendation from a person qualified to assess your academic work; clinical, public health, or professional experiences; or leadership potential in public health.

Note: ISP Certificate of Management Studies in Health Services Administration applicant’s letter should also include the applicant’s current role, as well as ability to access critical information needed to complete the units of study

Organizational Chart and Job Description

ISP Certificate of Management Studies in Health Services Administration applicants only.

Provide an organizational chart showing your current position within your organization along with your current job description.

Official Transcripts1

U.S.Institutions

Provide a separate official transcript from every U.S. college/university attended, including University Extensions. You must provide official transcripts even if the courses are not required for admission or transferred to another institution. This includes non-degree programs (e.g., summer school). Transcripts are considered official only if they arrive in our office in a sealed envelope from the college or university.

International Institutions

Applicants who have attended international universities that issue official transcripts/mark sheets on request must submit such credentials directly to the University of Minnesota, School of Public Health. The transcripts/mark sheets must be received in the School of Public Health directly from the schools. An official English translation should be attached if the documents are not in English. OFFICIAL transcripts/mark sheets bear the original signature of the Registrar or other appropriate university official, and the seal of the issuing university.

Applicants who have attended international institutions which do not issue transcripts, exam reports, or mark sheets, should submit an official statement from the Registrar or their adviser giving a detailed description of their program, a qualitative evaluation of their work, and the results of all exams taken.

All applicants who submit credentials on which the award of the degree received is not recorded should submit certified copies of their diplomas; an official English translation should be attached if the diploma was not issued in English.

If the grading system employed is not shown on the credentials themselves, a separate statement from the university is required giving this information.

On CERTIFIED or ATTESTED credentials, the certifying official should write, "This is a true copy of the original," and should then sign and seal each document. Applicants may not certify their own credentials.

Proof of English Language Proficiency

Applicants, whose native language is not English, are required to take the Test of English as a Foreign Language (TOEFL). An official report of the TOEFL scores should be sent to the School of Public Health. Institutional code: 8417. Department code: 50.

The preferred performance level for admission to the School of Public Health is a TOEFL score of 600 on the paper test or 250 on computer-based test or 100 on the internet based (iBT). Scores must be under two years old.

If you have completed 16-semester credits/ 24-quarter credits (within the past 24 months) in an academic program in a recognized institution of higher learning in the U.S., you do not need to submit the TOEFL as part of the application process.

If you have previously provided official transcripts to the University of Minnesota School of Public Health, you do not need to have these transcripts resent.

If you want the School of Public Health to give information to a second party or legal agent, such as a friend, relative, or organization, please list, under item 54 of the Application for Admission Form, the name of the person or organization with which the School of Public Health may share information. Providing fax or e-mail addresses on the application form will be taken as authorization to release information to those addresses. If you leave item 54 blank, the School of Public Health will not discuss your application with anyone other than those specified below.

All information on the Application for Admission Form is private. The University of Minnesota complies with federal and state privacy laws and regulations. Those who may gain access to information in your file are staff and faculty at the University who have a need to gain access, and outside organizations and government bodies in limited circumstances as authorized by state or federal law. In addition, you may review your own file. No one else may review your file without your written consent or a subpoena or court order.

All items followed by an asterisk (*) on the Application for Admission Form are required. Required items will be used for identification or admissions purposes. Information on this form may also be used for decisions on awarding assistantships, scholarships, and/or other financial support opportunities.

Gender and ethnicity information is needed for reporting and research purposes. All individual student information will be treated as confidential; it will not be used in a discriminatory manner. The information collected will be used for summary reports required by federal and state laws and regulations and by the University of Minnesota. Refusal to provide any of the requested information will not adversely affect the processing of your application.

Providing a birth date or former name, or Social Security number is voluntary. The School of Public Health uses this information for positive identification to insure that your records are not confused with those of other students. In the University of Minnesota’s computerized system, the Social Security number is the primary means of identification. Therefore, although your application will be considered without a Social Security number or birth date, providing these will reduce the likelihood of error when matching credentials or correspondence with your application.

Providing an e-mail address is optional. If you do not want to be contacted at your e-mail address, please do not provide the e-mail address. Please be aware that the School of Public Health cannot guarantee that e-mail information will be secure.
