Application Information

Application to the School is though SOPHAS, the Schools of Public Health Application Service. SOPHAS is a service of the Association of Schools of Public Health, which represents the 37 CEPH accredited schools of public health.


Supporting Documentation Requirements for Admission

All supporting documentation listed below should be provided directly to SOPHAS (except where noted).

1. Statement of Purpose and Objectives
Provide a essay in SOPHAS describing your past education, experience, and current professional career objectives. You are encouraged to comment on any or all of the following: plans you have to use your education and training; the needs and/or challenges you perceive as important in your field of study; and any personal qualities, characteristics, and skills you believe will enable you to be successful in your chosen field of study.

1A. Applicants to the PhD program in Health Services Research, Policy, and Administration must declare their preferred Area of Emphasis in their Statement of Purpose and Objectives personal essay. 

1B. Research Statement - Epidemiology PhD applicants only. Epidemiology PhD applicants must also provide a proposed research statement directly to the School of Public Health.

2. Résumé or C.V.
Upload directly in the appropriate section of the SOPHAS application.

3. References
Provide 3 letters of reference from persons qualified to assess your academic work; clinical, public health, or professional experiences; or leadership potential in public health. Letters of reference should be provided directly to SOPHAS.

4. Official Transcripts:  U.S. Institutions
Have an official transcript sent from every U.S. college/university attended directly to SOPHAS, including institutions where courses are not required for admission or transferred to another institution. This includes non-degree programs (e.g., summer school).

5. Official Transcripts:  International Institutions
The University of Minnesota, School of Public Health requires that applicants who have attended international universities that issue official transcripts/marksheets on request must submit such credentials directly to the University of Minnesota, School of Public Health. The transcripts/marksheets must be received in the School of Public Health directly from the schools. An official English translation should be attached if the documents are not in English. OFFICIAL transcripts/marksheets bear the original signature of the Registrar or other appropriate university official, and the seal of the issuing university.

Applicants who have attended international institutions which do not issue transcripts, exam reports, or marksheets, should submit an official statement from the Registrar or their adviser giving a detailed description of their program, a qualitative evaluation of their work, and the results of all exams taken.

All applicants who submit credentials on which the award of the degree received is not recorded should submit certified copies of their diplomas; an official English translation should be attached if the diploma was not issued in English.

If the grading system employed is not shown on the credentials themselves, a separate statement from the university is required giving this information.

On CERTIFIED or ATTESTED credentials, the certifying official should write, "This is a true copy of the original," and should then sign and seal each document. Applicants may not certify their own credentials.

*Standardized test scores are not required of applicants to the noted major programs who have completed a doctoral-level degree at a U.S. institution of higher learning.

For the following tests, please have test scores sent directly to SOPHAS:

  • GRE Designation: 5688 (or have them sent directly to the University of Minnesota School of Public Health using institution code "6874", department code "0616".)
  • MCAT Designation: SOPHAS
  • TOEFL Designation: 5688 (or have them sent directly to the University of Minnesota School of Public Health using institution code "8417", department code "50".)

For the following tests, please have official, certified or attested copies of test scores sent directly to the University of Minnesota, School of Public Health:

ECFMG provide official or certified copy

7. English Language Proficiency

Applicants whose native language is not English or whose education was completed exclusively at an institution(s) whose language of instruction is not in English must prove English proficiency by providing proof of one of the following:

  • Official TOEFL scores - An official report of the scores should be sent directly to SOPHAS. Designation: 5688. TOEFL scores must be under two years old.
  • Completion of 16-semester credits/ 24-quarter credits (within the past 24 months) in an academic program at a recognized institution of higher learning in the U.S. or Canada
  • USMLE scores or ECFMG certificate

8. Entry Terms and Application Deadlines

Applicants should be sure to note the appropriate entry term(s) listed below for the major or certificate program to which they are applying.

Applicants should be sure to note the appropriate priority (if any) and final deadlines noted below for submitting a complete application for admission (including the application fee in the correct amount and all supporting documents). To be considered for admission for a particular term, the application for admission and all supporting documentation must submitted to SOPHAS by the final deadlines noted below.

Priority deadlines are for scholarship opportunities and (where applicable) admission consideration.
