Regents' Certificate Application for Admission
Thank you for your interest in the University of Minnesota, School of Public Health (SPH). This application for admission information is for applicants to the following Regent’s Certificate programs only:
- Food Protection
- Preparedness, Response and Recovery
- Management Fundamentals in Healthcare Organizations
- Leading Integrated Health Systems

Application Fee
An application fee of $75.00 must accompany each application for admission. The application fee cannot be deferred and is not refundable. It should be sent in the form of a personal check or money order made payable to the University of Minnesota. International applicants should submit the fee in the form of a bank draft drawn on a U.S. bank, made payable (in U.S. dollars) to the University of Minnesota. Credit card or cash payments are not accepted.
Supporting Documents
The following supporting documents are required to complete the application for admission. Please have all documents sent directly to our office at the address noted above.
1. Statement of Purpose and Objectives
Provide an essay describing your past education, experience, and current professional career objectives. You are encouraged to comment on any or all of the following: plans you have to use your education and training; the needs and/or challenges you perceive as important in your field of study; and any personal qualities, characteristics, and skills you believe will enable you to be successful in your chosen field of study.
2. Résuméor C.V.
3. Letter of Recommendation
Provide one letter of recommendation from a person qualified to assess your academic work; clinical, public health, or professional experiences; or leadership potential in public health.
4. Official Transcripts – U.S. Institutions1
Provide a separate official transcript from every U.S. college/university attended, including University Extensions. You must provide official transcripts even if the courses are not required for admission or transferred to another institution. This includes non-degree programs (e.g., summer school). Transcripts are considered official only if they arrive in our office in a sealed envelope from the college or university.
5. Official Academic Credentials - International Institutions (if applicable)
Applicants who have attended international institutions should provide an official course-by-course credential evaluation by World Educational Services (WES). Have WES send the evaluation directly to the University of Minnesota School of Public Health.
Note:U.S. applicants, who have studied abroad as part of their coursework at a U.S. university/college, do not need to provide an evaluation of their coursework taken abroad as long as the international coursework and grades are noted on their U.S. university/college transcript.
6. Proof of English Language Proficiency (if applicable)
Applicants, whose native language is not English, are required to take the Test of English as a Foreign Language (TOEFL) or the International English Language Testing System (IELTS). An official report of the TOEFL or IELTS scores should be sent to the University of Minnesota-Minneapolis/St. Paul, institution code 6874.
The preferred performance level for admission to the School of Public Health is a TOEFL score of 600 on the paper test or 250 on computer-based test or 100 on the internet based (iBT). Additionally, the preferred minimum on the IELTS is 7.0 (total band score). Scores must be under two years old.
If you have completed 16-semester credits/ 24-quarter credits (within the past 24 months) in an academic program in a recognized institution of higher learning in the U.S., you do not need to submit the TOEFL as part of the application process.

If you want the School of Public Health to give information to a second party or legal agent, such as a friend, relative, or organization, please list, under item 54 of the Application for Admission Form, the name of the person or organization with which the School of Public Health may share information. Providing fax or e-mail addresses on the application form will be taken as authorization to release information to those addresses. If you leave item 54 blank, the School of Public Health will not discuss your application with anyone other than those specified below.
All information on the Application for Admission Form is private. The University of Minnesota complies with federal and state privacy laws and regulations. Those who may gain access to information in your file are staff and faculty at the University who have a need to gain access, and outside organizations and government bodies in limited circumstances as authorized by state or federal law. In addition, you may review your own file. No one else may review your file without your written consent or a subpoena or court order.
All items followed by an asterisk (*) on the Application for Admission Form are required. Required items will be used for identification or admissions purposes. Information on this form may also be used for decisions on awarding assistantships, scholarships, and/or other financial support opportunities.
Gender and ethnicity information is needed for reporting and research purposes. All individual student information will be treated as confidential; it will not be used in a discriminatory manner. The information collected will be used for summary reports required by federal and state laws and regulations and by the University of Minnesota. Refusal to provide any of the requested information will not adversely affect the processing of your application.
Providing a birth date or former name, or Social Security number is voluntary. The School of Public Health uses this information for positive identification to insure that your records are not confused with those of other students. In the University of Minnesota’s computerized system, the Social Security number is the primary means of identification. Therefore, although your application will be considered without a Social Security number or birth date, providing these will reduce the likelihood of error when matching credentials or correspondence with your application.
Providing an e-mail address is optional. If you do not want to be contacted at your e-mail address, please do not provide the e-mail address. Please be aware that the School of Public Health cannot guarantee that e-mail information will be secure.
