Wholey honored at MDH Fall Learning Day

SPH professor Doug Wholey was recognized this month by the Minnesota Department of Health for his contributions to the development of health care homes in the state.

A community of certified health care professionals honored Wholey at MDH’s fall learning day, “Health Care Homes at Year Five: Voices from the Emerging Future,” on Oct. 2.

Wholey, director of the Center for Care Organization & Development, made significant contributions to the creation of health care homes –an approach to primary care in which primary care providers, families and patients work in partnership to improve health outcomes and quality of life for individuals with chronic health care conditions and disabilities.
